Scope of Services:

The PHRM/ISS (Perinatal High Risk Management / Infant Services System) Program is established to help improve access to healthcare and to provide enhanced services to certain Medicaid-eligible pregnant/postpartum women and infants. The enhanced services for this target population includes case management, nursing assessment, psychosocial and nutritional counseling/ assessments, home visits and health educaiton. 

FHC PHRM/ISS Program Goal:

  • The main goal is to help reduce the infant mortality rate to no more than seven deaths per 1000 live births and to help reduce the low birth weight rate to no more than five percent of all births. The very low birth weight rate should be reduced to one percent of all births.

Family Health Center is a private, non-profit corporation 501(c)(3) which is governed by a community-based Board of Directors. The Board of Directors consists of members who use Family Health Center as their center for primary healthcare and who are community representatives.


This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
