What is a sliding fee scale?
A: The sliding fee scale system was developed to enable all FHC patients to participate in paying for their health care services based on their income. Discounts are offered based on family/household size and annual income.

A: Failure to report all available coverage, including primary, secondary, or supplemental insurance, may constitute misrepresentation or fraud.

When do I have to pay my co-pay, deductible, and co-insurance amount?
A: Payment for co-payments, deductibles, and co-insurance amounts is due at the time of service, unless prior arrangements have been made.

How Long Can I Stay on WIC?
A:

Infants can stay on WIC until they are 12 months old

Pregnant women can stay on WIC during pregnancy and up to six weeks after giving birth

Breastfeeding mothers can stay on WIC up to one year after giving birth

Mothers who are not breastfeeding can stay on WIC up until six months after giving birth

What if I am not satisfied with my eligibilty ro qualification decision?
A: If you are not satisfied with the decision about your eligibility or qualification, you have the right to appeal any decision. You may request, either orally or in writing, a fair hearing within 60 calendar days of the decision. A hearing will follow within one week of the request. Everyone involved will discuss and review the case. You may hire legal counsel to represent you at your expense. You will receive a decision in writing within 45 days of the original request. If you are still not satisfied, you may take the case to court at your expense.

What do I do about my WIC benefits if I move to a new state?
A: If you are planning to move to a new state, please let your WIC clinic know. The staff will give you information to take with you to ensure that you continue to receive your WIC benefits in your new state.
