As a nonprofit community health center, FHC is a Patient Centered Medical Home (PCMH) providing access to quality, comprehensive, and affordable primary and preventive healthcare to all residents in its five-county service area.
Our dedicated team of board-certified physicians, nurse practitioners, and other certified clinical support staff is your partner to achieve better health and well-being. We also partner with local specialists for patient referrals, when needed.
Quality care is our focus. Clinical and social outcomes are evaluated and reported to ensure optimal care, including prenatal care, childhood immunization, cancer screening, diabetes, and cardiovascular disease. Electronic health records help provide continuity of care at all FHC locations.
