Our physicians provide high quality, comprehensive primary preventive healthcare services. Our physicians are assisted by highly competent clinical staff, which consists of registered nurses, licensed practical nurses, medical assistants, and other certified staff.

Behavioral Health 

 

​FHC Behavioral Health Program is a service that provides individual and family counseling through a qualified healthcare provider. Behavioral healthcare for people ages 5 and up is served at FHC’s Main Clinic, which offers a caring staff of professionals, a comfortable environment, and private sessions.

Personalized professional services for a wide range of common behavioral health conditions affecting adults, seniors, children and adolescents, such as:

  • ADHD

  • Alzheimer’s disease

  • Anxiety Disorders

  • Obsessive Compulsive Disorder

  • Schizophrenia

  • Substance Abuse Disorder

Coping with today’s fast-paced world often presents challenges.

welfare or safety, rely on the behavioral health specialist at FHC.

Allergy Testing:

Utilizing the PCMH Model, FHC provides comprehensive primary care and preventive medicine from aged 5 and older.

 

Services Include:

  • Percutaneous skin test​

Diabetes Management

 

Utilizing the PCMH Model, FHC provides comprehensive primary care and preventive medicine from ages 17 and older.

 

Services Include:

Smoking Cessation 

FHC provides a certified smoking cessation program which includes:

  • Educational programs provided by certified tobacco treatment specialist 

  • Group therapy sessions

  • Available cessation aids

PHRM/ISS Program

The PHRM/ISS (Perinatal High Risk Management / Infant Services System) Program is established to help improve access to healthcare and to provide enhanced services to certain Medicaid-eligible pregnant/postpartum women and infants. The enhanced services for this target population include case management, nursing assessment, psychosocial and nutritional counseling/ assessments, home visits and health education. 

FHC PHRM/ISS Program Goal:

  • The main goal is to help reduce the infant mortality rate to no more than seven deaths per 1000 live births and to help reduce the low birth weight rate to no more than five percent of all births. The very low birth weight rate should be reduced to one percent of all births.

Family Health Center is a private, non-profit corporation 501(c)(3) which is governed by a community-based Board of Directors. The Board of Directors consists of members who use Family Health Center as their center for primary healthcare and who are community representatives.

 

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
