Behavioral Health
FHC Behavioral Health Program is a service that provides individual and family counseling through a qualified healthcare provider. Behavioral healthcare for people ages 5 and up is served at FHC’s Main Clinic, which offers a caring staff of professionals, a comfortable environment, and private sessions.

PHRM/ISS Program
The PHRM/ISS (Perinatal High Risk Management / Infant Services System) Program is established to help improve access to healthcare and to provide enhanced services to certain Medicaid-eligible pregnant/postpartum women and infants. The enhanced services for this target population include case management, nursing assessment, psychosocial and nutritional counseling/ assessments, home visits and health education.
FHC PHRM/ISS Program Goal:

The main goal is to help reduce the infant mortality rate to no more than seven deaths per 1000 live births and to help reduce the low birth weight rate to no more than five percent of all births. The very low birth weight rate should be reduced to one percent of all births.
