Family Health Center's Hypertension Heroes!
OB/GYN Clinic and Pharmacy Department Win Big in 2025 Challenge
In a challenge rooted in care, commitment, and community health, two Family Health Center departments rose above the rest.
Congratulations to the OB/GYN Clinic and the Pharmacy Department, the standout winners of FHC’s 2025 Hypertension Control Challenge, a clinic-wide competition focused on lowering average blood pressure levels across patient populations.
Over the two-month challenge period (November–December), departments raced not for trophies, but for impact: improving the lives of hypertensive patients by helping them reach a target BP of 139/89 mmHg or better.
The OB/GYN Clinic led among Health Support Teams, leveraging consistent care, re-checks, and proactive patient follow-up to deliver meaningful results. Meanwhile, the Pharmacy Department topped the Patient Support category, turning education into action through counseling, medication guidance, and collaboration with referring departments.
The result? Real progress. Better numbers. Healthier patients.
As one of the challenge’s core goals, this effort showed what’s possible when departments don’t just treat conditions—they empower change. And though a team lunch (courtesy of Ambetter Health) is on the table, the bigger win is what it means for FHC patients: lower risks, longer lives, and a system that works together to lift outcomes for all.
Here’s to the teams who proved that hypertension control isn’t just a metric—it’s a mission.
Winners:
-
OB/GYN Clinic – Health Support Team Champion
-
Pharmacy Department – Patient Support Team Champion
Let’s keep the momentum going. Every reading matters. Every patient counts
